Assessment of Carpal Tunnel Syndrome

Carpal tunnel syndrome occurs when the median nerve is compressed at the wrist. Symptoms may include tingling or numbness in the thumb, index and middle fingers, night symptoms and weakness. Similar symptoms can come from other conditions, so examination and, where appropriate, nerve tests help guide treatment.

When surgery may be considered

Options depend on symptom duration, severity, examination and evidence of nerve damage. A wrist splint, activity adjustments or a selected injection may be discussed. Surgery may be considered when symptoms persist or when nerve impairment makes earlier treatment appropriate. Progressive numbness or weakness should receive timely medical assessment.

Carpal tunnel release

Release surgery divides the ligament forming the roof of the carpal tunnel to reduce pressure on the nerve. The anaesthetic and approach are chosen individually. The operation aims to relieve nerve compression but cannot guarantee full restoration of sensation or strength. Longstanding or severe nerve damage may limit recovery, and some symptoms may have another cause.

Risks of surgery

Risks include bleeding, infection, scar tenderness, altered sensation, injury to nerves or other structures, persistent symptoms, recurrence and further surgery. Pain, stiffness and reduced hand function can occasionally be prolonged. Anaesthetic risks depend on the method and your health. Discuss your personal risk and the consequences of delaying treatment before deciding.

Recovery and hand use

Your team will advise on dressings, wound care, finger movement and hand elevation. Hand therapy may be needed. Work, driving, lifting and tool use depend on your job, strength and healing. Nerve symptoms may recover at a different rate from the wound. Follow-up is important to assess function and address persistent concerns.
